Storyline

One year after the events of Flight 180, Kimberly Corman and her friends are driving to Daytona Beach for spring break. While stopped at a highway on-ramp, Kimberly has a vivid premonition of a horrific multi-car pile-up initiated by a logging truck losing its chains, resulting in widespread destruction and numerous fatalities, including herself and her friends. Snapping back to reality, she realizes the events are about to unfold. Panicked, she blocks the on-ramp with her SUV, preventing several cars from entering the highway, including a mother and son, a stoner, a businessman, a pregnant woman and her husband, and a lottery winner. A police officer, Thomas Burke, arrives and questions her, but just as he's about to arrest her, the premonition's events terrifyingly come true behind them, with the logging truck causing the massive accident. While many die on the highway, Kimberly and the others who were prevented from entering are saved. However, Death, feeling cheated, begins to hunt down the survivors in the order they would have died in the premonition. The first survivor to die is the stoner, Shaina, whose car is struck by a speeding truck after her tire blows out. The lottery winner, Evan, dies in his apartment in a fire started by a dropped cigarette, then falls from his window onto a fire escape ladder which impales him. Kimberly seeks help from Clear Rivers, the sole survivor of Flight 180, who is now voluntarily committed to a psychiatric hospital, believing she is safe from Death there. Clear explains that Death's design works in reverse; to break the chain, a new life must interrupt Death's cycle. They deduce that Isabella, the pregnant woman, is key, as her baby could be the new life. As the group tries to protect Isabella, more horrific and elaborate 'accidents' occur: Nora, the mother, is decapitated by an elevator door, and Kat, the businesswoman, is impaled through the head by an airbag after a log from the initial accident re-enters her car. Clear herself is killed trying to save Eugene, the police officer who was burned, after an oxygen tank explosion. The group believes that Isabella giving birth will save them. During an attempt to rush Isabella to the hospital, she is pulled over by another officer and gives birth prematurely in a field. The baby is healthy, but Death's cycle hasn't been broken. They realize that the new life must be born into the 'design' itself, meaning a near-death experience, not just a literal birth. Kimberly remembers that she drowned in the premonition but was revived. To create a 'new life' scenario, she drives the ambulance into a lake, drowning herself. Officer Burke pulls her out, and she is successfully resuscitated, believing she has cheated Death. Later, at a celebratory barbecue, it is revealed that one of the survivors, Brian, the son of the mother and son pair, was nearly hit by a news van earlier, thus having his own 'new life' experience. However, at the barbecue, Brian is suddenly obliterated by an exploding barbecue grill, revealing that the chain of death continues.
